Advanced Practitioner- Physician Assistant or Nurse Practitioner to join well established Health Centers-
Positions available in the Waddington Health Center, Heuvelton Health Center and the Canton Health Center

Position Requirements:

Enthusiastic patient-centered care champion to provide excellent quality care to our patients, ranging from pediatrics through geriatrics
Must be motivated, engaged and committed to providing high quality care and be operationally driven to achieve optimal productivity and efficiency
Strong interpersonal communication skills with ability to interact with all types of individuals within a wide range of personality traits and socioeconomic statuses
Services provided: Annual physicals, sick visits, chronic disease management, vaccinations, lab testing to name a few
Willingness to work and live in rural, small community setting; and actively engage with community
PA/NP works under the supervision of a physician. However, may be the principal care provider onsite. In such cases, the PA/NP confers with the supervising physician and other medical professionals as needed and as required by law.

Offer Package Outline:

Starting Salary Range: $120,000 per year with adjustments based on experience, plus an additional WRVU-based incentive bonus.
